During the initial complimentary (free) consultation with Dr. Kalantarian Plastic Surgery (Dr K Plastic Surgery in O.C.) for a breast augmentation, the patient’s desire will be determined in detail, a medical history will be obtained followed by examination of the breasts by doing a set of measurements that include but not limited to size, shape, the skin quality of the breasts, linear distances from the sternal notch to the nipples and to the infra-mammary fold (IMF = where breast meets the chest wall), nipple to nipple distance as well as circumference of the chest. Presence or absence of Ptosis (sagginess) will depend on the location of the nipple with reference to IMF and breast parenchyma. The difference between Saline, Silicone and Gummy Bear (cohesive gel) will be described to the patient. Depending on the patient’s desire, the detail measurements and patient’s evaluation and Dr Kalantarian’s experience in breast enhancement a recommendation is made. After all the patient’s questions are answered, a recommendation will be made; different breast implants will be tested on the patient’s chest with the patient looking in a mirror. It is important to determine the relative width of the breast compared to the diameter of the implant, chest circumference, patient height and weight when making a decision about the implant size, shape and profile.

Pre-operative Period.
After initial consultation, a pre-operative test date will be set within 7-10 days from the date of surgery. The operative consents with be reviewed with the patient, Dr Kalantarian will meet with the patient again to answer any questions discuss risk, benefit and alternative options and order routine pre-op laboratory tests. Pre-operative instructions are given when the surgery date is determined. A mammogram may be ordered depending on the age and family history of the patient. The patient needs to avoiding any substance that could thin the blood 10 days before and 3 weeks from the date of surgery (e.g. aspirin or ibuprofen containing products, alcohol, Vitamin E, Fish oil, etc). Smoking has to be stopped long enough before and after the surgery (smoking can increase potential pulmonary and cardiac complications and it can interfere with wound healing as well as increased risk of scar tissue formation around the implant or capsular contraction).
One day before the surgery:

- Time and exact location of the surgery center.
- Arrive at the center one to one and half hours before the surgery start time.
- Dress comfortably (e.g. loose clothing, no blue jeans, flat shoes) and avoid wearing any jewelry.
- Shower or bath the night before the surgery.
- Do not applying any lotion, oil or cream on the day of surgery.
- No food or drink is allowed about 10 hours before the surgery
- If you are taking medications, bring a list of the medications with you to the surgery center. You need to ask the doctor to see which Medication to take before the surgery.
- You must have a ride from and to the surgery center.
- Someone should be with you for 48 hours following the surgery.

What is involved before and during the procedure?

Dr Kalantarian, breast augmentation surgeon, normally performs this type of procedure in an outpatient surgery facility unless the patient has a contra-indication. Before the procedure, you will be taken to the preoperative holding room. The nurse will greet you and ask you routine medical questions. A board certified anesthesiologist will also interview and evaluate you. The anesthesiologist will also tell you about the type and nature of the anesthesia that will be used. Dr Kalantaian prefers to perform the breast augmentation procedure under general anesthesia. Dr Kalantarian will meet you, take photos if no photos were taken during your pre-op testing period, will mark on your chest different land marks that are needed as well as location of the incision.
The incision can be around the areola, in the breast fold or axillary (armpit) region. The incision along the areola and the breast fold allow the surgeon to visualize the entire pocket while is made (direct approach); however, the axillary approach without the use of an endoscope is considered a blind approach. Majority of the patients prefer the areola or breast fold incision.
After achieving general anesthesia, the area will be prepped and draped in a sterile fashion. The Fresno breast augmentation surgeon will make an incisions in areas that will leave minimal visible scars with the patient’s consent which will be decided during the consultation and confirmed before the surgery. Decision about the incision are usually based on the type of the breast implant (saline or silicone) that will be used, diameter of the areola, the patient’s anatomy and the preference of the patient and surgeon. The implants will then be inserted under the pectoral muscles in majority of the cases. When the implant is placed under the muscle the breast feels and looks more natural and the potential risk of scar tissue formation around the breast implant is significantly less.
After completion of the procedure, the incisions will be covered with Steri-strip tapes, dressed, and chest will be wrapped. You will be moved to recovery room where you will spend about 45 minutes to an hour. After your full recovery, you will be discharged to home with your ride.
Instructions for after the procedure:
You will be given detail post operative instruction after the surgery. A local pain medication is injected in the breast pocket during the surgery and IV pain medication will be given in the recovery room that keeps you comfortable for 6 hours or more. You will be given a prescription for pain, antibiotics and nausea. You may experience pain and discomfort in the chest after the surgery. In order to minimize the pain, we encourage you to take one pain medication 40 minutes after the nausea medication every 6 hours for the first 1-2 days. You may take another pain medication as needed every 4 hours (need to read and follow the instructions for each pain medication). Usually the bruising on the breast or chest is minimal or none. If there is any bruising it will disappear within 3 weeks. Keep your head of bed elevated with 3-4 pillows and sleep on your back not side. You need to avoid any activity that increases your heart rate or blood pressure for about 2-3 weeks. However, you need to walk every 2 hours after the surgery for about 3 minutes to minimize risk of blood clot formation (except night while asleep).
